To refer a patient to Central Coast Physical Therapy please fill out the following referral form and fax to:

Once we receive the referral form we will contact the patient to schedule an appointment within 24 hours or when convenient for the patient.
Once the Patient has been evaluated by a physical therapist; you should expect a comprehensive evaluation report faxed to your office within 24 hours. When applicable, you can also expect regular updated progress reports to be faxed to your office.
Our office will verify the patient's physical therapy insurance benefits. If you are referring a patient claiming workers compensation, we will contact the adjuster and request physical therapy authorization. In the case of referring a patient claiming workers' compensation, it is appreciated if the adjuster's name, phone number, and all other related information be faxed to our office along with the physical therapy referral.
